Passion Youth Engage 2018 Youth Camp - Waiver and Rules Release Information

Medical Information and Liability Release Form:

I being 18 years of age or older, do for myself and for and on behalf of my participant, if child is not 18 years of age or older do hereby release, forever discharge and agree to hold harmless Eagle Heights Church from any and all liability, claims, discovered now or in the future, or demands for personal injury, emotional illness, sickness or death, including those injuries that could not be foreseen or were not foreseeable, as well as property damage and expenses, of any nature whatsoever which may be incurred by the undersigned and the participant that occur while child is participating at Passion Youth Camp. Furthermore, I and on behalf of my participant if under the age of 18 years, hereby assume all risk, whether known or unknown, foreseeable of personal injury, sickness, death, damage and expenses as a result of participation in recreation and activities involved therein. Further, authorization and permission is hereby given to Eagle Heights to furnish any necessary transportation, food and lodging for this participant. I further agree to hold harmless Eagle Heights its employees and volunteers, for any liability sustained by Eagle Heights as the result of the negligent, willful or intentional acts of said participant, including expenses incurred attendant thereto. I am the parent or legal guardian of this participant, and grant my participant, and grant my permission for him/her to participate fully at Passion Youth Camp, and give my permission to take said participant to a doctor or hospital and hereby authorize medical treatment, including but not in limitation to emergency surgery or medical treatment and assume the responsibility of all medical bills. Further, should it be necessary for participant to return home due to medical reasons, disciplinary action or otherwise, I agree to resolve the matter.

Camp Rules:

We have rules at camp so that we can honor the Lord in all we do, and so everyone can have the best possible camp experience! We seek to honor the Lord by creating and emphasizing a culture of respect and honor. We expect the youth to honor God by honoring the camp facilities, leaders, and their peers. We will have grace for the teens, because we know we are all growing in the Lord. However, blatant disrespect is not acceptable. Use of foul language and destruction of property are also examples of other behavior not permitted at Passion Youth Camp.

Modesty is also necessary to create a camp environment that honors the Lord. The following rules have been established to uphold our standard of modesty: All shorts must be fingertip length. Shirts may be sleeveless, but must not have spaghetti straps. Also, shirts and shorts must be worn by all youth (boys and girls) when swimming. Our night services are normally casual, but if dresses/skirts are worn they must be knee length or longer.

It is our desire for the youth to commit themselves to hearing from God. In order to limit distractions by disconnecting from social medial and outside influences, youth are not allowed to bring cell phones to camp. Parents can still get in touch with their children at any time through our camp leaders. Their numbers will be provided on the info cards given out before camp.


The full camp payments for your child must be paid by June 22, 2018. If there are any issues speak with Pastor Josh or Sarah. All Deposits are non-refundable after the date of June 4, 2018. Any refunds must be requested before this date or a refund will not be given.

Service Time and Location

-Services will begin at 10:00 am as usual.

-Services will be located outdoors behind the gym.

Parking Note

-Please try to reserve the parking lot located next to gym for senior adults and handicapped parking.

What you need to bring

-A Friend! We would be delighted to have the opportunity to minister to your friends and family, so please invite someone and bring them with you.

-A chair, a blanket if you would like to sit on the ground, even a “pop-up” is welcomed.


-While we won’t have any children’s church services, we will have our nursery open for children 0-3. If will only be open from 9:45 – 11:00 am during the service. We will need you to pick up your children as soon as service is over (no later than 11:00 am) in order to allow our nursery workers the opportunity to enjoy the remainder of the event.

What do I wear?

-Feel free to dress casual. Shorts are fine. Modesty please!

-We’re asking all VBS volunteers and children to wear their VBS shirts, if they have one.

Free Food…Come dine with us!

-Hamburgers, Hot Dogs, Baked Beans, Chips, and Sweet Tea! As American as it gets!

-Lunch will be served from 11:15 – 12:00 pm.

-For dessert we will have independently owned vendors bringing an Ice Cream Truck and Snowballs! Prices range from $1- $3 so bring cash!

Fun for all!!

-Indoor Inflatables (12 and under) on the north side of the gym from 12:00 – 1:00 pm.

-Water Slides (12 and under) from 12:00 – 2:00 pm.

Girls will need to wear a bathing suit with a t-shirt and shorts; boys will need to wear bathing suit bottoms and a shirt. Don’t forget a towel and change of clothes if you don’t want them to ride home wet.

*Please make a note: Once children are wet, we are asking that they not be allowed to enter into the gym area for any reason. The floors get extremely slippery when wet and we wouldn’t want anyone to get hurt. Also we cannot allow the dry inflatables to get grass or mud inside of them. The rental company would not like that at all. The gym bathrooms will still be accessible from the outside of the building on the south end.

-Volleyball (ages 13 and up) on the south side of the gym from 12:00 – 1:00 pm.

-Softball - - - There will be 3 “pick-up” games: 12:00-12:40 pm (ages 19 and up), 12:45-1:25 pm (ages 13-18) and 1:30-2:10 pm (ages 19 and up). If you’d like to play, please bring your glove and bat if you would like.

**Parents please remember that everyone will be responsible for their own children at this particular event.

We are seeking to keep our volunteer involvement to a bare minimum to allow them time to relax and enjoy themselves. We are asking you to do your best and to work together to help keep our children in order and respectful of one another and the inflatable equipment so that everyone can have a great time! We are planning on baptizing those who have decided to follow Christ and are ready to obey Jesus’ commandment to follow Him in believer’s baptism!

We will be hosting our annual Fall Festival Celebration on campus Sunday evening October 27th from 5:00 to 8:00 pm. The celebration is open to everyone and free of charge. Our address is 47318 Rufus Bankston Rd. in Tickfaw, LA 70466. 

Our Fall Festival Celebration will feature something fun for the entire family. We will have a carousel for young children, pony rides, a trackless train ride, inflatables (bouncers, slides, and a bungee run), a rockwall, and a playland for todlers. We will also have concession stands that will be open throughout the event featuring cotton candy, and a candy station for kids where they can come and go throughout the night freely.

Church members are encouraged to volunteer and exemplify the love of Christ through service as we reach out to our community to provide a fun-filled and entertaining evening of Christian fellowship. Church members can sign up in the foyer on Sunday mornings or Wednesday nights, before or after service, begining Sunday October 6th.
